| Title | Visual outcomes of scleritis-associated intraocular inflammation compared to isolated scleritis |

| Purpose | The role of intraocular inflammation associated with scleritis, spectrum of ocular complications and associated visual morbidity requires better understanding. We evaluate the visual outcomes, ocular complications and therapies for patients with scleritis-associated intraocular inflammation (SAI) compared to isolated scleritis (IS). |
| Methods | Fifty-two patients with a diagnosis of anterior or posterior scleritis, 36 with SAI and 16 with IS, with at least 3 months follow-up were retrospectively included. Demographic data, ocular and medical history, associated systemic diseases and treatments were recorded. Baseline and interval office visits were assessed for visual acuity, scleritis location and activity, uveitis, and ocular complications. Visual outcomes, ocular complications and therapies for SAI and IS were compared. |
| Results | Mean age was 48.4(+/- 15.4) and 53(+/- 17.1) years old(p=0.37) and 19(53%) and 9(56%) were women(p=0.82) in the SAI and the IS groups, respectively. Systemic diseases tended to be more frequently diagnosed for patients with IS(p=0.07). The need for immunosuppressive treatment did not differ in the 2 groups during follow-up(p=0.16) or at last visit(p=0.49). Patients in the SAI group developed more posterior segment complications than in the IS group(p=0.002). Visual loss was worse for patients with SAI compared to IS(p=0.04). |
| Conclusion | SAI was associated with a higher rate of visual morbidity compared to IS. The majority of patients in both groups required systemic immunosuppression. Patients with SAI may warrant more aggressive therapy given the higher rate of visual loss and ocular comorbidities. |
