TitleJanus kinase inhibitors, a new potential weapon in refractory scleritis and nonspecific orbital inflammation: a case series.

PurposeTo highlight Janus kinase inhibitors (JAK-i) as a promising treatment for isolated orbital inflammatory disorders, presenting the first use of JAK-i in non-specific orbital inflammation (NSOI) and baricitinib in isolated scleritis, with the longest follow-up reported for JAK-i in isolated ocular inflammation.
MethodsCase series of two patients with isolated NSOI and refractory anterior scleritis. Comprehensive workups excluded infectious, rheumatological, and (para)neoplastic causes. JAK-i was initiated due to inflammation unresponsive to several DMARDs. Primary outcome: inflammation control; secondary outcome: adverse events.
ResultsA 28-year-old female with isolated NSOI received tofacitinib as first-line, steroid-sparing treatment due to condition severity. A 46-year-old male with isolated anterior scleritis, previously treated with multiple DMARDs, received baricitinib. Both showed clinical improvement without relapses or serious side effects during a median follow-up of 21 months (range: 18–24 months).
ConclusionThis study demonstrates the first successful use of JAK-i in NSOI and baricitinib in isolated scleritis. JAK-i may be effective for refractory and severe isolated autoimmune NSOI and scleritis, with extended follow-up offering insights into long-term efficacy and safety. Collaboration between uveitis specialists and rheumatologists is essential for optimal outcomes.
