Efficiency of Spectral -Domain Coherence Tomography in the detection of retinal macroaneurysm due to diabetic macula edema in routine practice.

Purpose
: To evaluate the performance and efficiency of SD-OCT in the detection of retinal macro aneurysms (RAM)in patients with diabetic macula edema (DME) secondary to diabetic retinopathy
Methods
A retrospective review of the charts of patients who presented with DME undergoing DR at the Pitié Salpetrière Hospital, Paris, France, between April 2019 and April 2022. Subjects underwent Fluorescein angiography (FA) and Spectral -Domain Optical Coherence Tomography (SD-OCT). Heidelberg Engineering, Germany. To investigate the prevalence of RAM in eyes with DME, FA, and SD-OCT images were qualitatively reviewed by four independent observers (two graders per group). The Kappa coefficient of Cohen was calculated to determine agreement between the graders
Results
The kappa coefficient of Cohen, for inter-rater agreement in evaluating FA images was 0.444, indicating a moderate agreement between the first and second-raters. Thus, FA's sensitivity and specificity for the RAM diagnosis were 73% and 55%, respectively. If macroaneurysm was assessed with SD-OCT image analysis, the kappa coefficient for inter-rater agreement was 0.850, implying an acceptable agreement between both readers. The sensitivity and specificity of SD-OCT were 91% and 56% respectively.
Conclusion
This study analyzed and showed the usefulness of SD-OCT, a noninvasive and rapid exam that remarkably describes the RAM as a round oval hyperreflective lesion in the inner retina layer. To our knowledge, this is the first report showing the usefulness of SD-OCT in detecting RAM in practical real life.
