Bacterial endogenous chorioretinitis and intermediate uveitis after gynecological surgery: case series report

Purpose
To describe the clinical and OCT features of patients with bacterial endogenous chorioretinits and intermediate uveitis after gynecological surgery.
Methods
3 patients with diagnosis of bacterial endogenous chorioretinitis and intermediate uveitis were included in case series.
Results
3 patients came separately within month, they were previously treated in another eye hospitals. All 3 patients had bilateral intermediate uveitis and multifocal chorioretinitis. All insisted that their condition was related to previous gynecological surgery and her visual problems appeared at day 3 after surgery. The surgery were different, but were performed by the same surgeon and anesthesiologist at the same operating room under general anesthesia in the same day. None of them had any gynecological inflammatory complications. They had postoperative good general condition, and the only systemic complaint was a pustular rash on head during the first two weeks. Visual acuity ranged from 20/200 to 20/20 depending on whether the foveola was affected. The chorioretinal lesions varied from small to massive confluent lesions equal to several diameters of the optic nerve disc. The OCT shows subretinal lesions originating at the retinal pigment epithelium/choroid layer and penetrating into the neurosensory retina and vitreous with destruction of the pigment epithelial layer. All patients had ESR increase, 2 had positive ASO-test and 1 patient had staphylococcus aureus in blood. All had positive dynamic with AB-therapy with methylprednisone.
Conclusion
This case series demonstrates rare case of endogenous bacterial chorioretinisis and intermediate uvetis (not endoftilmitis) after gynecological surgery.
