TitleClinical and imaging features of posterior nodular scleritis and large posterior choroidal granuloma

PurposeTo study and compare baseline clinical and swept source-optical coherence tomography (OCT) features of posterior unifocal - nodular scleritis (NS) with those of large unifocal choroidal granuloma (CG).
MethodsRetrospective, cross-sectional study to compare baseline clinical and imaging characteristics of NS and CG groups.
ResultsNS group comprised 11 eyes (10 cases) while CG 10 eyes (10 cases). CG had mean age 28.7 years ±6.94 compared to NS 41.70 years ±11.88. Inflammatory markers (outer retinal disruption, double layer sign, vitreous hyper reflective dots) were noted commonly in eyes with CG. Structural changes in inner retina (ILM and inner retinal folds) and exudative changes (intraretinal fluid, subretinal fluid) were comparable in both groups. Choroid-scleral interface (CSI) at the edge of NS lesion showed anterior bowing or ‘up the hill’ appearance while CG lesions showed posterior bowing or ‘down the hill’ appearance.
ConclusionOCT diagnostic markers such as direction of CSI at the edge of lesion along with inflammatory markers in vitreous and outer retina are helpful adjuvants in differentiation of CG lesions from NS.
